Independent Departure Authorization Form

At DanSci Limited, safeguarding is our primary concern, and we are committed to ensuring the safety of all our dancers. We understand the frustrations that can arise from waiting in line to sign out children, especially after a long performance event.

To address this, we are offering an option for dancers aged 14 and above to leave independently, with your consent.

Please complete this form if you wish to allow your child to depart from the performance event on December 8th without a parent/guardian present.

By signing this form, I, the parent/guardian of the above-named dancer:

Give permission for my child to leave at the end of the performance event independently on December 8th, 2024.
Understand that DanSci Limited's duty of care and responsibility for my child ends once they have left the premises.
Acknowledge that I have discussed safety measures with my child for traveling home alone, especially considering the early darkness in December.
Release DanSci Limited from all responsibility and liability once my child has left the event premises.
Confirm that my child is capable of traveling home safely on their own and has a means of contacting me if needed.
Understand that this permission is valid only for the event on December 8th, 2024, and that separate authorization will be required for future events.

This form must be submitted to DanSci Limited 6pm the event on December 8th.
Dancers without a completed form will not be allowed to leave independently.
DanSci Limited reserves the right to refuse independent departure if we have concerns about a dancer's safety.
